Church Registration

If you have made the decision to offer CSB programs in your church, or are considering doing so, we’re looking forward to partnering with you in growing godly men! These forms are part of the registration process for your church. Every church offering CSB programs must complete a Working Agreement with us, so you’ll want to start there. Next, we need current information about your church and a list of the leaders serving in your CSB programs. We also ask each church offering CSB programs to partner with us financially so we can continue to offer great ministries to boys and young men, be able to revise and improve our material, and launch new initiatives for you to use in your ministry!

After completing the following forms you may submit them electronically, by fax or by mail to CSB Ministries Canada as soon as possible to help launch your ministry:

Working Agreement

As part of our ministry partnership with local churches we ask each church to sign the working agreement which includes shared responsibilities and our Standard of Faith.

Church Information Update

We will use this information to update our records concerning your church and programs. Please note the programs and fees listed, and the changes that have been made for this year.

Leader Membership Form

This form should list all active leaders of your programs. and will insure that our staff can contact the leaders in your groups.

Partners Together

Determine and fill out your annual financial commitment. Generous financial support from your local unit fuels the engine to take this movement to more churches in the coming year. Through your financial and prayer support, together we cast the vision to more men and women.
